To request copies of your medical records please complete the Authorization to Disclose Medical Information form. Link to form. Make sure the form is signed and filled out completely. Forms may be mailed or faxed. A fee may be charged if the release is for something other than continuing care.
Disability Forms
Effective May 1, 2010, St. Paul Heart Clinic will charge $20.00 to complete any disability, return to work or FMLA forms for patients being followed at St. Paul Heart Clinic. Payment is due at the time of the request.
